Description:
Provide skilled physical therapy interventions under the direction and supervision of a licensed Physical Therapist in accordance with the established plan of care.
Assist patients in improving mobility, strength, balance, endurance, safety, and functional independence within the home environment.
Monitor patient response to treatment and communicate progress, concerns, and recommendations to the supervising Physical Therapist.
Educate patients, family members, and caregivers regarding home exercise programs, safety strategies, fall prevention, and use of adaptive equipment.
Travel throughout the designated service area to provide patient care in homes and community settings.
Collaborate with the interdisciplinary home health team, including nursing, occupational therapy, speech therapy, social work, and home health aides, to optimize patient outcomes.
Maintain accurate, timely, and compliant documentation according to agency, state, federal, and payer requirements.
Participate in staff meetings, case conferences, quality improvement initiatives, and continuing education opportunities.
Home health experience preferred but not required.
