Job SummaryWe are seeking a skilled Therapy/Rehabilitation professional to help patients recover from injuries, illnesses, or disabilities and regain independence. The role involves evaluating patient needs, developing treatment plans, and delivering therapeutic interventions to improve mobility, function, and overall quality of life.
Key Responsibilities- Assess patients’ physical, cognitive, and functional abilities
- Develop individualized rehabilitation and treatment plans
- Implement therapy interventions (exercise programs, manual therapy, functional training, ADL support)
- Monitor patient progress and adjust care plans as needed
- Educate patients and caregivers on exercises, recovery strategies, and injury prevention
- Maintain accurate documentation in EMR systems
- Collaborate with physicians, nurses, and multidisciplinary care teams
- Ensure compliance with healthcare regulations and safety standards
- Assist patients with mobility, strength, coordination, and daily living activities
- Participate in discharge planning and continuity of care
Required Qualifications- Bachelor’s, Master’s, or Doctorate degree in a therapy-related field (e.g., Physical Therapy, Occupational Therapy, Rehabilitation Science)
- Active state licensure/certification relevant to discipline (PT, OT, etc.)
- Strong understanding of rehabilitation techniques and patient care practices
- Excellent communication and interpersonal skills
- Ability to create and implement effective care plans
- Basic computer skills and familiarity with EMR systems
Preferred Qualifications- Clinical experience in rehabilitation, acute care, or outpatient settings
- Experience with specialized populations (geriatrics, neurological, orthopedic, pediatric, or post-surgical patients)
- Certifications in specialized rehab areas (e.g., neuro rehab, sports rehab, hand therapy)
- Experience in interdisciplinary team environments
