Key Responsibilities of the Physical Therapist (Home Health)
Conduct higher-level visits such as SOC, evaluations, reassessments, and discharges
Document visits within various EMR systems including Kantime, Kinnser (WellSky), Axxess, and Homecare Homebase
Coordinate visit scheduling directly with patients
Provide care across multiple home health agency environments
Utilize central scheduling platform: Pariox EMR
Requirements of the Physical Therapist (Home Health)
Valid Physical Therapy license in the state of Florida
Experience in home health settings
Proficiency in EMR systems
