Seven Healthcare is seeking an experienced Physical Therapist for a travel assignment in Albemarle, NC. This role focuses on home health services, providing care to adult patients with various chronic diseases and post-orthopedic surgery needs. The position offers a dynamic work environment where you will schedule and conduct patient visits, working closely with a supportive clinical team.

Key Responsibilities of the Physical Therapist
  • Conduct up to six home health visits per day
  • Develop individualized patient care plans to achieve optimal functional outcomes
  • Administer gait and balance training, and manage pain without pharmaceuticals
  • Communicate effectively with a multidisciplinary team to prevent patient readmissions

Requirements of the Physical Therapist
  • Valid driver's license and proof of insurance
  • BLS certification and state licensure required
  • Experience in geriatric/pediatric, chronic disease management, and OASIS-E
  • Minimum of 1-year experience in Home Health
  • Proficiency in Home Care Home Base EMR
