Seven Healthcare are seeking a qualified Physical Therapist for a travel assignment located in Ashland, KY. This role focuses on home health services, using your expert skills to create and implement therapy plans for clients to achieve their highest possible level of function and independence.

Key Responsibilities of the Physical Therapist
  • Perform physical therapy evaluations and treatments based on physician referrals
  • Conduct comprehensive evaluations using appropriate tools and procedures
  • Review patient records for relevant medical history, precautions, and contraindications
  • Establish measurable goals and develop treatment plans for maximum patient independence
  • Regularly reassess treatment effectiveness and patient progress
  • Communicate patient needs and progress with the treatment team and medical providers
  • Recommend and aid in securing adaptive equipment for enhanced rehabilitation
  • Maintain and submit required documentation in adherence with regulations
  • Supervise Physical Therapist Assistants and Aides in patient-related activities
  • Participate in internal staff development and continuing education

Requirements of the Physical Therapist
  • KY Physical Therapist Licensure, Compact Licensure Accepted
  • 1 year of home health physical therapy experience
  • Previous OASIS experience
  • Familiarity with home care home base software
