Minimum Qualifications

• Associate degree in Physical Therapy from an accredited program.
• Active Illinois Physical Therapist Assistant license required.
• Active American Heart Association BLS required.
• Minimum 2 years of PTA experience required.
• Strong background in Total Knee Replacement required.
• Total Hip Replacement experience preferred.
• Experience in a busy outpatient clinic preferred.
• Candidates must be located outside the facility’s 50-mile radius; local candidates within that radius will not be considered.
• Completed work history and skills checklist current to 1 year, pertinent to Physical Therapy Assistant required.
• Two supervisory reference forms completed with dates employed listed required.

• Facility Type: Hospital (Outpatient Physical Therapy).
• Shift Name: Days.
• Shift Schedule: 4x8, 08:00–16:30.
• Shift Notes: Ability to work 4x10s if preferred; 40 hour guarantee.
• Weekend Requirement: No weekends.
