Key Responsibilities

• Deliver physical therapy treatments to patients in a Skilled Nursing Facility (SNF) setting under the supervision of a licensed Physical Therapist
• Assist in implementing individualized treatment plans to support patient recovery and functional goals
• Monitor and document patient progress, reporting changes in condition to the supervising Physical Therapist
• Educate patients and caregivers on therapeutic exercises and safe mobility techniques

Required Qualifications

• Active Physical Therapy Assistant (PTA) license in the state of Illinois
• Graduate of an accredited Physical Therapist Assistant program
• Current CPR/BLS certification
• Experience working in a clinical or rehabilitative care setting

Preferred Experience / Skills

• Prior experience working in a Skilled Nursing Facility (SNF) environment
• Familiarity with geriatric patient populations and long-term care rehabilitation protocols
