Key Responsibilities

• Deliver physical therapy treatments to patients in a skilled nursing facility (SNF) setting under the supervision of a licensed Physical Therapist
• Assist in implementing individualized patient care plans in accordance with therapy goals
• Monitor and document patient progress, reporting updates to the supervising Physical Therapist
• Educate patients and caregivers on therapeutic exercises and home care techniques

Required Qualifications

• Active Physical Therapy Assistant (PTA) license in the state of Minnesota or ability to obtain prior to start
• Graduate of an accredited Physical Therapist Assistant program
• Current CPR/BLS certification
• Prior experience working in a skilled nursing facility (SNF) or long-term care setting

Preferred Experience / Skills

• Experience with geriatric patient populations
• Familiarity with electronic medical records (EMR) documentation systems
