Key Responsibilities

• Provide physical therapy assistant services to patients in a home health setting across an assigned geographic service area
• Travel to patient homes within the designated service pod for scheduled visits
• Document patient care and progress using HCHB (Homecare Homebase) electronic medical records software
• Collaborate with the broader therapy and clinical team to support patient treatment plans and goals
• Track and report mileage exceeding 20 miles for billing purposes between patient visits

Required Qualifications

• Active Physical Therapy Assistant (PTA) license in the state of California
• Minimum 1–2 years of experience as a PTA
• Proficiency with or ability to learn HCHB (Homecare Homebase) documentation platform
• Valid driver's license and reliable transportation for home visits

Preferred Experience / Skills

• Prior home health experience strongly preferred
• Familiarity with the Walnut Creek, Lafayette, or Contra Costa County service areas
• Comfort working independently in a field-based, patient-facing environment
