Key Responsibilities- Deliver physical therapy treatments to patients in a Skilled Nursing Facility (SNF) setting under the supervision of a licensed Physical Therapist
- Implement individualized treatment plans to improve patient mobility, strength, and functional independence
- Monitor and document patient progress, reporting changes in condition to the supervising Physical Therapist
- Educate patients and caregivers on home exercise programs and safe movement techniques
- Collaborate with interdisciplinary care teams to ensure coordinated patient care
Required Qualifications- Active Physical Therapy Assistant (PTA) license in the state of Texas
- Graduate of an accredited Physical Therapist Assistant program
- Current CPR/BLS certification
- Experience working in a Skilled Nursing Facility (SNF) setting preferred
Preferred Experience / Skills- Prior experience in a SNF or long-term care environment
- Familiarity with electronic medical records (EMR) documentation systems
