Key Responsibilities- Provide physical therapy assistant services to patients in a home health setting
- Complete approximately 30 visits per week within an assigned territory in the Indianapolis area
- Document patient care accurately and in a timely manner using HCHB electronic medical records
- Collaborate with the supervising Physical Therapist to implement and progress individualized treatment plans
- Maintain compliance with home health regulations and organizational policies
Required Qualifications- Active Physical Therapist Assistant (PTA) license in the state of Indiana
- Minimum of 12 months of home health experience within the past 3 years
- Proficiency with HCHB (HomeCare HomeBase) electronic medical records
- Ability to manage a caseload of approximately 30 visits per week
- No on-call requirements
Preferred Experience / Skills- Familiarity with Indianapolis-area home health service territories
- Strong time management and organizational skills in a field-based care environment
