Key Responsibilities

• Provide physical therapy assistant services to patients in a home health setting
• Complete approximately 30 visits per week within an assigned territory in the Indianapolis area
• Document patient care accurately and in a timely manner using HCHB electronic medical records
• Collaborate with the supervising Physical Therapist to implement and progress individualized treatment plans
• Maintain compliance with home health regulations and organizational policies

Required Qualifications

• Active Physical Therapist Assistant (PTA) license in the state of Indiana
• Minimum of 12 months of home health experience within the past 3 years
• Proficiency with HCHB (HomeCare HomeBase) electronic medical records
• Ability to manage a caseload of approximately 30 visits per week
• No on-call requirements

Preferred Experience / Skills

• Familiarity with Indianapolis-area home health service territories
• Strong time management and organizational skills in a field-based care environment
