Key Responsibilities- Provide physical therapy assistant services to patients in a home health setting across a multi-county territory
- Meet a minimum productivity expectation of 30 points per week
- Complete all clinical documentation by 10:00 AM the following day
- Deliver patient care across assigned service areas within the designated territory
Required Qualifications- Active Physical Therapist Assistant (PTA) license
- Minimum of one year of recent home health PTA experience
- Proficiency with HCHB (Homecare Homebase) documentation software
- Ability to travel throughout a multi-county service territory
Preferred Experience / Skills- Familiarity with home health regulatory and documentation requirements
- Strong time management skills with the ability to manage a high-volume caseload independently
