Key Responsibilities
- Provide physical therapy assistant services across two facility locations within the same area
- Deliver skilled rehabilitation services to a high-acuity skilled nursing caseload focused on return-to-home outcomes
- Provide therapy services to long-term care residents as well as skilled patients
- Collaborate with supervising Physical Therapists to implement individualized treatment plans
- Maintain accurate and timely clinical documentation in accordance with facility and regulatory standards
Required Qualifications
- Active Physical Therapy Assistant (PTA) license in the state of Illinois
- Graduate of an accredited Physical Therapist Assistant program
- Ability to work across multiple facility locations
- Strong clinical documentation skills
Preferred Experience / Skills
- Prior experience in a skilled nursing facility (SNF) or long-term care (LTC) setting
- Experience with high-volume or fast-paced rehabilitation caseloads
- Familiarity with return-to-home rehabilitation protocols
