Key Responsibilities

• Provide physical therapy assistant services across two facility locations within the same area
• Deliver skilled rehabilitation services to a high-acuity skilled nursing caseload focused on return-to-home outcomes
• Provide therapy services to long-term care residents as well as skilled patients
• Collaborate with supervising Physical Therapists to implement individualized treatment plans
• Maintain accurate and timely clinical documentation in accordance with facility and regulatory standards

Required Qualifications

• Active Physical Therapy Assistant (PTA) license in the state of Illinois
• Graduate of an accredited Physical Therapist Assistant program
• Ability to work across multiple facility locations
• Strong clinical documentation skills

Preferred Experience / Skills

• Prior experience in a skilled nursing facility (SNF) or long-term care (LTC) setting
• Experience with high-volume or fast-paced rehabilitation caseloads
• Familiarity with return-to-home rehabilitation protocols
