Key Responsibilities- Provide physical therapy assistant services to patients in a home health setting
- Implement treatment plans established by the supervising Physical Therapist
- Document patient progress and communicate updates to the care team
- Travel to patient homes within the assigned service counties to deliver care
Required Qualifications- Active Physical Therapy Assistant (PTA) license
- Prior home health experience required
- Ability to work a full-time Monday through Friday schedule, 40 hours per week
- Reliable transportation and ability to travel within the assigned service area
Preferred Experience / Skills- Familiarity with multi-county home health service delivery
- Strong documentation and communication skills in a home-based care environment
