    Physical Therapy Assistant (PTA) – Home Health

    Schedule / Job Type

    Contract | Days | Full-Time | 5x8-Hour Shifts (8:00 AM – 4:30 PM) | Onsite / Field-Based

    Key Responsibilities
    • Provide skilled physical therapy assistant services to patients in a home health setting across the East Portland area and surrounding communities
    • Visit up to 6 patients per day, maintaining a 1:1 care model with standard productivity expectations
    • Complete clinical documentation using EPIC Remote Client for Home Health and the Rover mobile application, including OASIS assessments
    • Manage daily scheduling and travel efficiently across an assigned patient territory, with occasional drive times of up to 60–90 minutes per day
    • Collaborate with the supervising physical therapist and care team to support patient treatment plans and progress
    Required Qualifications
    • Active or pending Physical Therapy Assistant (PTA) license in the state of Oregon
    • Minimum of 1 year of skilled home health experience
    • Current Basic Cardiac Life Support (BCLS) certification
    • Valid driver's license and reliable personal transportation
    • Ability to work a 5x8 day shift schedule (no 4x10 or 3x12 availability)
    Preferred Experience / Skills
    • Familiarity with EPIC, Rover, and/or OASIS documentation platforms
    • Experience managing an independent caseload in a home health or community-based setting
    • Strong time management and self-directed work skills suited for field-based care delivery
