Deliver physical therapy treatment services to patients in a skilled nursing facility (SNF) setting under the supervision of a licensed Physical Therapist
Implement individualized treatment plans as established by the supervising Physical Therapist
Document patient progress, treatment sessions, and clinical outcomes in accordance with facility and regulatory standards
Communicate effectively with the interdisciplinary care team regarding patient status and progress
Required Qualifications
Active Physical Therapy Assistant (PTA) license in the state of Minnesota or eligibility to obtain
Graduate of an accredited Physical Therapist Assistant program
Experience working in a skilled nursing facility (SNF) setting preferred
Current CPR/BLS certification
Preferred Experience / Skills
Prior experience in a SNF or long-term care environment
Familiarity with electronic medical records (EMR) documentation systems
