Key Responsibilities
- Deliver physical therapy treatment to residents in a skilled nursing facility setting under the supervision of a licensed Physical Therapist
- Implement individualized treatment plans and document patient progress accurately and in a timely manner
- Assist in the assessment and modification of therapy programs to meet the evolving needs of patients
- Collaborate with interdisciplinary care teams to support patient rehabilitation goals
Required Qualifications
- Active Physical Therapy Assistant (PTA) license in the state of Connecticut or eligibility for licensure
- Graduate of an accredited Physical Therapist Assistant program
- Current CPR/BLS certification
- Experience working in a skilled nursing facility (SNF) setting
Preferred Experience / Skills
- Prior travel or contract therapy experience
- Familiarity with electronic medical records (EMR) systems used in SNF environments
