Schedule / Job Type

Contract | Full-Time | Days | Monday – Friday, 8:00 AM – 4:30 PM | Optional weekend on-call rotation (1 weekend per month)

Key Responsibilities

• Provide physical therapy assistant services to patients in a home health setting across an assigned geographic territory
• Complete patient visits in accordance with a productivity standard of 6.4 visits per 8-hour day (32 visits per 40-hour week)
• Travel to patient homes within the designated service area surrounding the Torrington region
• Participate in a brief office orientation during the first two weeks of assignment
• Participate in optional weekend on-call rotation, approximately once per month

Required Qualifications

• Active Physical Therapy Assistant (PTA) license in good standing
• Minimum of 1 year of home health experience
• Ability to travel within approximately 40 miles of the Torrington, CT area
• Reliable transportation for daily patient home visits

Preferred Experience / Skills

• Experience with Home Care Home Base (HCHB) electronic medical records system
• Familiarity with the northwest Connecticut region, including surrounding towns
