Contract (13 Weeks) | Full-Time | Days | Monday – Friday, 8:00 AM – 4:30 PM | Onsite / Field-Based

Key Responsibilities

• Provide physical therapy assistant services to patients in a home health setting across an assigned geographic territory
• Complete a target of 6.4 patient visits per 8-hour day (32 visits per 40-hour week), with each visit counted as one point
• Travel to patient homes within approximately a 40-mile radius of the base office location
• Complete required orientation, including a minimum of 2 days in the office during the first two weeks of the assignment
• Participate in optional weekend on-call rotation, approximately one weekend per month

Required Qualifications

• Active Physical Therapy Assistant (PTA) license in the state of Connecticut
• Minimum of 1 year of home health experience
• Ability to manage an independent caseload across a defined geographic service area
• Reliable transportation and willingness to travel within the assigned territory

Preferred Experience / Skills

• Experience with Home Care Home Base (HCHB) electronic medical records system
• Familiarity with the northwestern Connecticut region, including surrounding towns and communities
