Schedule / Job Type

Contract | Full-Time | Days | Monday–Friday with weekend rotation approximately every 6 weeks | Onsite / Field-Based (Home Health)

Key Responsibilities

• Provide physical therapy assistant services to patients in a home health setting within an assigned territory
• Conduct and document patient visits using HCHB electronic medical records system
• Complete Start of Care assessments as required
• Meet productivity expectations of 28–30 points per week
• Participate in weekend rotation coverage as scheduled
• Attend a minimum 2-day in-office orientation prior to field deployment

Required Qualifications

• Active Physical Therapy Assistant (PTA) license in the state of Connecticut
• Prior home health experience required
• Experience with HCHB (Homecare Homebase) EMR required
• Start of Care experience required

Preferred Experience / Skills

• OASIS experience preferred
• Familiarity with field-based, territory-driven care delivery models
