Schedule / Job Type

Contract | Full-Time | Days | Monday – Friday with weekend rotation approximately every 6 weeks

Key Responsibilities

• Provide physical therapy assistant services to patients in a home health setting within an assigned territory
• Perform and document patient visits using the HCHB electronic medical records system
• Meet productivity expectations of 28–30 points per week
• Conduct Start of Care assessments as required
• Participate in a weekend rotation schedule as assigned

Required Qualifications

• Active Physical Therapy Assistant (PTA) license
• Prior home health experience required
• Experience with HCHB (Homecare Homebase) EMR system
• Experience with Start of Care documentation
• Ability to travel within an approximate 40-mile radius of the assigned office location

Preferred Experience / Skills

• OASIS experience strongly preferred
• Comfortable working independently in a field-based, patient home environment
