Key Responsibilities

• Conduct comprehensive patient evaluations and develop, implement, and modify individualized treatment plans in alignment with patient needs, goals, and physician direction
• Perform 60-minute evaluations and treatments, designing functional programs that support effective return to community activities
• Complete thorough and timely medical record documentation including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
• Ensure all care is delivered under valid physician orders and within insurance authorization requirements
• Serve as an educator for patients, families, students, and coworkers; participate in student programs by providing supervision, developing learning objectives, and completing student evaluations
• Adhere to discipline-specific State Practice Act and all applicable regulatory standards while maintaining professional licensure in good standing

Required Qualifications

• Active Physical Therapist Assistant (PTA) license in good standing
• Outpatient physical therapy experience required — new graduates will not be considered
• Proficiency in clinical documentation, goal writing, and compliance with state and federal guidelines
• Demonstrated commitment to patient and employee safety practices

Preferred Experience / Skills

• Experience with 60-minute evaluation and treatment sessions in an outpatient setting
• Active involvement in continuing education and professional development activities
• Familiarity with performance improvement methodologies and quality of care initiatives
