Contract (13 Weeks) | Full-Time | Days | Monday – Friday, 8:00 AM – 4:30 PM | Onsite / Field-Based

Key Responsibilities

• Provide physical therapy assistant services to patients in a home health setting within an assigned geographic territory
• Complete a minimum of 6.4 patient visits per 8-hour day, targeting 32 visits within a standard 40-hour work week
• Travel to patient homes within approximately 40 miles of the office location to deliver care
• Participate in a voluntary weekend on-call rotation, approximately once per month
• Complete a minimum 2-day in-office orientation within the first two weeks of assignment

Required Qualifications

• Active Physical Therapy Assistant (PTA) license in the state of Connecticut
• Minimum of 1 year of home health experience
• Ability to travel within a 40-mile radius of the Watertown, CT office
• Valid driver's license and reliable transportation

Preferred Experience / Skills

• Experience with HomeCare HomeBase (HCHB) electronic documentation system
• Familiarity with home health visit productivity standards and point-based scheduling models
