Job Title: Occupational Therapist

Location: Josephine County, OR

Job Duration: 3 Months with possibility of extensions
Facility: Assured Home Health – Grants Pass
Shift: Monday–Friday, 8:00 AM – 4:30 PM
Weekly Hours: 36 hours guaranteed

Position Overview

Assured Home Health is seeking a dedicated and skilled Occupational Therapist to provide high-quality, patient-centered care in a home health setting. The ideal candidate will be experienced in evaluating and treating patients to improve their ability to perform daily activities, while maintaining compliance with clinical and regulatory standards.

Key Responsibilities

• Conduct comprehensive Occupational Therapy (OT) evaluations for patients in a home health setting
• Perform routine and follow-up therapy visits
• Complete Start of Care (SOC) assessments when required
• Manage recertifications, resumptions of care, and patient discharges
• Develop, implement, and adjust individualized treatment plans
• Document patient care thoroughly and accurately in compliance with agency and regulatory guidelines
• Collaborate with interdisciplinary team members to ensure continuity of care

Required Qualifications

• Active Oregon Occupational Therapist (OT) license
• Current CPR certification
• Ability to work independently and manage a caseload efficiently
• Strong documentation and clinical assessment skills

Preferred Qualifications

• Prior home health experience
• Strong time management and organizational skills
• Excellent communication and patient care skills

Compensation & Additional Details

• Weekly hour guarantee: 36 hours
• Mileage reimbursement provided (from first patient to last patient)
• No on-call or weekend requirements

Work Environment & Dress Code

• Home health setting across Josephine County
• Business casual or scrubs
