Physical Therapist (PT) – Home Care

Weekly Hour Guarantee: 40 hours Productivity Expectations: 30 points Coverage Areas: Morris County & Essex County, NJ Shift Schedule: Monday–Friday, 8:00 AM–4:30 PM On‑Call / Weekend Requirement: One weekend per month, if possible Mileage Reimbursement: Yes — reimbursed between visits, from first patient to last patient Pay Rate: $49/hr

Top Responsibilities

• Evaluate and treat patients in their homes according to individualized care plans
• Develop PT treatment plans based on clinical assessments and patient goals
• Document patient progress accurately and in compliance with agency standards
• Coordinate with interdisciplinary teams to ensure comprehensive patient care
• Educate patients and caregivers on exercises, safety, and functional mobility

Required Qualifications

• Active NJ PT License — New Jersey PT licensing requirements
• Minimum 2–3 years of experience in home care, acute care, or subacute rehab
• Strong assessment, treatment, and clinical decision‑making skills
• NJ PT License
