• Home Health Admissions: Conduct comprehensive home health admission assessments to evaluate patient status, safety, and need for skilled therapy services.
• Evaluations & Care Planning: Perform thorough physical therapy evaluations and establish individualized plans of care aimed at improving mobility, strength, and functional independence.
• Follow-Up Visits: Execute routine follow-up visits, providing skilled therapeutic interventions and monitoring patient progress toward goals.
• Discharge Assessments: Complete accurate and timely discharge assessments and documentation upon the completion of the care plan.
• Care Coordination: Collaborate effectively with an interdisciplinary healthcare team to ensure coordinated and safe patient-centered care.
• Provider Communication: Proactively communicate with the patient's primary provider and the care team regarding status changes, progress, or safety concerns.

• Education: Graduate of an accredited Physical Therapy program.
• Licensure: Current and active Physical Therapist license in the state of assignment.
• Certification: Current CPR certification (BLS – Healthcare Provider).
• Logistics: Valid driver’s license, reliable transportation, and active auto insurance.
