Job Title: Physical Therapist- Home Health/Hospice
Location: Grants Pass, OR
Contract: 3 months contract with contract extensions
Shift: Mon-Fri 8:00am -16:30pm

What are the top responsibilities for this position? (Please be as detailed as possible): Physical Therapy Visits - SOC, PT eval, Recert, ROC, supervisory visits
Must have qualifications/experience: Doctorate Physical Therapy
What skills/attributes are nice to have, and will set a candidate apart? Home Health experience, HomeCare Homebase EMR experience
Dress code/scrub color: business casual or any color of scrubs
