• Perform Start of Care (SOC), Resumption of Care (ROC), and Discharge evaluations
• Develop and implement individualized treatment plans based on patient needs
• Provide skilled physical therapy interventions in a home health setting
• Supervise and provide clinical oversight for Physical Therapist Assistants (PTAs)
• Accurately document patient care using EMR systems (HCHB preferred)
• Educate patients and caregivers on treatment plans, exercises, and safety
• Collaborate with interdisciplinary healthcare team members to ensure comprehensive care
• Monitor patient progress and adjust plans as necessary
  

• Active Physical Therapist (PT) license
• Home Health experience (required)
• Experience with EMR systems, specifically Homecare Homebase (HCHB) preferred
• Strong clinical assessment and documentation skills.
