• Patient Care: Deliver skilled physical therapy treatments in home health settings to improve patient mobility, function, and overall well-being.
• OASIS Assessments: Complete accurate and timely OASIS assessments to evaluate patient status, track progress, and determine appropriate care plans.
• Home Exercise Programs (HEP): Develop, implement, and educate patients and their caregivers on personalized HEP routines to support long-term recovery.
• Documentation: Maintain accurate, timely, and compliant clinical documentation of patient progress, interventions, and functional outcomes.
• Care Coordination: Collaborate with the interdisciplinary healthcare team to ensure continuous, safe, and effective patient-centered care.

• Education: Minimum of a Bachelor's degree in Physical Therapy.
• Experience: Minimum of 1 year of professional experience in a home health setting.
• Assessment Skills: Demonstrated, hands-on experience completing OASIS assessments.
• Licensure: Active and unrestricted Physical Therapist license/certification.
