    Location: The Villages and surrounding areas, FL
    Job Type: Full-Time / Contract
    Shift/Schedule: Monday – Friday, 8:00 AM – 4:30 PM (On-call/Weekend requirements apply)
    Weekly Hours: 40 Hours Guaranteed
    Mileage Reimbursement: Reimbursed for travel between visits, from the first patient of the day to the last patient.
    Pay Rate: $49/hr
    Job Overview
    We are seeking a highly skilled and adaptable Physical Therapist to provide expert care to patients within the community of The Villages and surrounding areas. This role focuses on delivering physician-ordered physical therapy services in the home setting, ensuring all care is provided in accordance with federal, state, and agency regulations. As a mobile clinical resource, you will manage a caseload focused on improving functional independence and safety for medically complex patients.
    Key Responsibilities
    • Evaluations & Care Planning: Perform comprehensive physical therapy evaluations and ongoing reassessments to identify functional limitations, safety risks, and the need for skilled interventions. Develop and implement individualized plans of care.
    • Skilled Interventions: Provide therapeutic exercises, gait and transfer training, balance activities, stair training, and home safety education.
    • Documentation & Compliance: Accurately document evaluations, visits, and progress notes in the Electronic Medical Record (EMR) in a timely manner. Adhere to all CMS Conditions of Participation and infection control standards.
    • Interdisciplinary Coordination: Collaborate with physicians, nurses, and other therapy disciplines (OT/ST) to ensure continuity of care. Identify and communicate changes in patient condition or barriers to progress promptly.
    • Patient Education: Instruct patients and caregivers on home exercise programs (HEP), fall prevention, the use of assistive devices, and energy conservation strategies.
    • Caseload Management: Efficiently manage a mobile caseload while meeting productivity expectations and adapting to different branch operations and workflows.
    Required Qualifications
    • Education: Graduate of an accredited Physical Therapy program.
    • Licensure: Current, active Physical Therapist license in the state of Florida.
    • Certification: Current CPR/BLS certification.
    • Experience: Minimum of 1 year of clinical experience as a Physical Therapist.
    • Technical Skills: Proficiency with electronic documentation systems and mobile technology.
    • Logistics: Valid driver’s license, reliable transportation, and active auto insurance; willingness to travel within the assigned coverage area.
