Shift schedule- Monday through Friday, 8AM- 5PM eastern time
Milage will be reimbursed between visits, from first patient to last patient
This job will have the following responsibilities:
Qualifications & Requirements:
This job will have the following responsibilities:
- Conducting Start of Care (SOC) visits
- Performing initial patient evaluations
- Managing routine revisits and follow-ups
- Completing re-evaluations to track progress
- Executing patient discharge planning and documentation
Qualifications & Requirements:
- Bachelor’s Degree in Physical Therapy (required)
- Prior experience with Home Care Home Base (HCHB) / PointCare systems
- Active Physical Therapist license (assumed standard requirement)
