Location: Palestine, TX

Weekly Guarantee: 36 hours Shift: Monday–Friday, 8:00 AM–4:30 PM On‑Call/Weekend Requirements: None Mileage Reimbursement: Yes — reimbursed between visits, from first patient to last patient Coverage Area: Crockett, Grapeland, Elkhart, Palestine Pay Rate: $47-$48/hr

About the Role

We are seeking a dedicated Travel Physical Therapist to provide high‑quality home health services across multiple communities. This role is ideal for a clinician who thrives in the field, values autonomy, and is comfortable managing a diverse caseload.

Key Responsibilities

• Perform comprehensive physical therapy evaluations
• Complete Start of Care (SOC) assessments
• Conduct 30‑day reassessments
• Provide treatment visits as clinically indicated
• Manage discipline discharges
• Complete agency discharges
• Deliver patient‑centered care in accordance with home health regulations and best practices
• Document accurately and efficiently within agency systems

Required Qualifications

• Active Physical Therapy License (state‑appropriate)
• Experience in home health
• Proficiency with HCHB (Homecare Homebase) or similar EMR systems
• Scrubs in Grey, Black, or Purple
