Travel Physical Therapist – Contract

Weekly Hour Guarantee: 40 hours Productivity Expectation: 6 points per day Location: Pensacola, FL Coverage Areas: Gulf Breeze, Navarre, Fort Walton Beach, Destin Mileage Reimbursement: Yes (from first patient to last patient) Shift Schedule: Mon–Fri 8:00am–4:30pm (Full Time) Dress Code: Royal blue scrubs OR docker‑style pants with a polo shirt Pay Rate: $47-$48/hr

About the Role

We are seeking a skilled Travel Physical Therapist to provide high‑quality home health services throughout the Gulf Breeze to Destin region. This role requires strong autonomous clinical judgment, excellent documentation skills, and proficiency with OASIS. As a key member of the home‑health team, you will deliver therapeutic interventions in patient homes while ensuring compliance with all clinical standards and care plans.

Key Responsibilities

Clinical Excellence:

• Deliver skilled physical therapy interventions in patient homes
• Provide therapeutic exercises, mobility training, gait training, and functional rehabilitation
• Ensure safe patient handling and fall‑prevention strategies

Patient Assessment:

• Perform comprehensive assessments using OASIS
• Develop, implement, and update individualized plans of care
• Evaluate patient progress and adjust treatment plans accordingly

Documentation:

• Complete accurate, timely, and compliant documentation for all visits
• Maintain OASIS proficiency and ensure regulatory accuracy

Education:

• Educate patients and caregivers on home exercise programs, safety, mobility, and functional improvement
• Provide clear instructions to support long‑term independence

Care Coordination:

• Communicate with the interdisciplinary team regarding patient status
• Collaborate with physicians, nurses, and other clinicians to ensure continuity of care

Adaptability:

• Manage a daily productivity expectation of 6 points
• Travel between multiple patient homes each day across the Gulf Breeze–Destin area

Required Qualifications

• OASIS proficiency required
• Home health experience preferred
• Active Florida Physical Therapist License (preferred; strongly advantageous)
• Strong clinical reasoning and assessment skills
• Personable, adaptable, and able to work independently
• Comfortable with daily travel and multiple home visits
