Schedule: Monday – Friday, 8:30 AM – 5:00 PM
Generations Home Health is seeking a skilled and dependable Physical Therapist (PT) to provide high-quality, patient-centered care across multiple locations in the Norfolk, Virginia Beach, and Portsmouth areas. This role requires strong clinical expertise in home health settings and the ability to work independently while maintaining flexibility.
Key Responsibilities
- Perform admissions, evaluations, and comprehensive assessments
- Conduct recertifications and discharge assessments
- Provide routine physical therapy visits in a home health setting
- Develop and implement individualized treatment plans
- Maintain accurate and timely documentation
- Collaborate with interdisciplinary teams to ensure optimal patient outcomes
Qualifications & Requirements:
- Licensed Physical Therapist (PT) with valid state license
- Home Health experience
- Ability to perform full-cycle patient care (admissions through discharge)
- Strong clinical judgment and documentation skills
