Travel PPV Physical Ther

Location: Renton, WA (South King County Coverage)
Type: Contract (36-hour weekly guarantee)
Schedule: Monday – Friday, 8:00 AM – 4:30 PM

Position Overview

We are seeking a dedicated Travel Physical Therapist to provide high-quality home health care services across the South King County area. In this role, you will evaluate functional status, develop comprehensive plans of care, and deliver evidence-based treatments to help patients regain independence and manage pain within the comfort of their homes.

Key Responsibilities

• Patient Assessment: Evaluate patients' functional status and body systems as required by CMS documentation to determine therapy needs.

• Plan of Care: Consult with physicians to develop and implement physical therapy plans of care; complete evaluations and report outcomes within 24 hours.

• Clinical Treatment: Treat patients to relieve pain and restore/maintain maximum function using evidence-based techniques.

• Collaboration: Instruct and inform family members, caregivers, and the healthcare team regarding progress and functional limitations.

• Supervision: Provide oversight for Physical Therapy Assistants (PTAs) and Home Health Aides in accordance with state regulations and LHC policy.

• Documentation: Prepare written progress notes for each visit and complete discharge summaries or status reports every 60 days.

Required Qualifications

• Education: Bachelor’s Degree (Minimum requirement).

• Licensure: Current Physical Therapy license in the state of Washington (or a valid Compact License).

• Certifications: Current CPR/BLS certification.

• Transportation: Valid driver’s license, vehicle insurance, and access to a dependable vehicle for travel between patient visits.

• Screening: Must pass required occupational health screenings.

Preferred Skills & Experience

• Previous Home Health experience.

• Proficiency with HomeCare HomeBase software.

• Strong leadership skills and the ability to work independently in a field-based environment.

Compensation & Logistics

• Travel: Mileage reimbursement provided for travel between patient visits.

• Dress Code: Solid color scrubs (no prints).

• Flexibility: While the standard schedule is M-F, some flexibility is required to meet patient demands, including potential weekend rotations or holiday availability.
