Pride Health is hiring a
Registered Nurse – Home Health for a 14-week contract assignment supporting a leading healthcare organization in
Denver, CO.Responsibilities Include:- Provide skilled nursing care to home health patients, including wound care, catheter management, medication management, and patient education.
- Perform Start of Care (SOC) visits and complete OASIS assessments and documentation.
- Manage patient care plans, coordinate services, and monitor patient progress as an RN case manager.
- Collect and manage labs while providing ongoing support and education to patients and families.
Qualifications:- Active Registered Nurse (RN) license required.
- Home health nursing experience required.
- Experience completing OASIS assessments and Start of Care visits required.
Important Details:Assignment Length: 14-week contract
Schedule: Monday–Friday, 8:00 AM–4:30 PM
Hours: 8-hour shifts / 40 hours per week
On-Call Requirement: Rotating on-call schedule
Coverage Area: Denver Metro Area
Mileage Reimbursement: Yes, mileage reimbursed between patient visits
Pay Range: $52.00–$54.00/hour
