Registered Nurse (RN) – Home Health/Hospice

Schedule: Monday–Friday, 8:30 AM–4:30 PM EST Weekly Hour Guarantee: 40 hours Productivity Expectation: 30 points Coverage Areas: Fayette County & Jessamine County On‑Call / Weekend Rotation: One weekend per month + two on‑call days per week Mileage Reimbursement: Yes — reimbursed between visits (from first patient to last) Pay rate: $48-$49/hr

Position Overview

The Home Health/Hospice RN is responsible for delivering high‑quality, patient‑centered nursing care in the home setting. This role includes comprehensive assessments, ongoing clinical evaluations, and coordination of care to support patients and families throughout the continuum of home health and hospice services.

• Start of Care (SOC) assessments
• Resumption of Care (ROC)
• Evaluations and Recertifications
• Routine skilled nursing visits
• Development and implementation of individualized care plans
• Documentation of all care in accordance with agency and regulatory standards
• Collaboration with interdisciplinary team members
• Patient and family education

Required Qualifications

• Active Registered Nurse (RN) license
• Current BLS certification
• Previous Home Health or Hospice experience (required
• Experience with HCHB (Homecare Homebase) or PointCare EMR systems
• Strong clinical assessment and critical‑thinking skills
• Excellent communication and time‑management abilities
