Job Summary

CareTenders of Needham is seeking a dedicated and compassionate Travel Registered Nurse (RN) to deliver high-quality home health services. The RN will be responsible for patient assessments, care planning, clinical interventions, and coordination with interdisciplinary teams to ensure effective and patient-centered care in a home setting.

Work Schedule

• Days: Monday – Friday
• Shift: 8:00 AM – 4:30 PM
• Weekly Hours Guaranteed: 32
• On-Call / Weekend Requirement: Yes

Key Responsibilities

• Develop, implement, and revise individualized patient care plans in collaboration with physicians and healthcare professionals
• Conduct comprehensive patient assessments (OASIS), including medication reconciliation
• Deliver skilled nursing care in accordance with the plan of care and infection control standards
• Supervise and support Home Health Aides (HHAs) and Licensed Practical/Vocational Nurses (LPNs/LVNs), including required supervisory visits
• Provide care for post-acute, assisted living, and nursing home patients in a home health setting
• Ensure accurate, timely, and compliant documentation of patient visits per agency policies and payer requirements
• Maintain timely documentation synchronization per company guidelines
• Collaborate with patients, families, and healthcare teams to ensure continuity and quality of care

Required Qualifications

• Active and valid Registered Nurse (RN) License – Massachusetts
• Strong clinical assessment and patient care skills
• Ability to adhere to infection control and regulatory standards
• Excellent documentation and communication skills

• Quick learner with the ability to rapidly onboard and adapt
• Experience with post-acute care, assisted living, or nursing home populations

• Mileage Reimbursement: Yes (between patient visits, first to last patient)
• Travel Requirement: Local travel within assigned coverage areas
