Description:

Title: Registered Nurse (RN) – Home Health
Location: Needham Heights MA

(Coverage Areas: Arlington, Belmont, Lexington, Lincoln, Sudbury, Waltham, Watertown, Wayland, Weston, Winchester (Massachusetts)

Duration: 3 months with possible contract extensions

Work Schedule

  • Days: Monday – Friday
  • Shift: 8:00 AM – 4:30 PM
  • Weekly Guarantee: 32 hours
  • On-call/Weekend Requirement: Yes

Job Summary

We are seeking a motivated and compassionate Travel Registered Nurse (RN) to provide high-quality home health care services. This role involves delivering patient-centered care, developing care plans, conducting assessments, and coordinating with interdisciplinary teams to ensure optimal patient outcomes in a home-based setting.


Key Responsibilities

  • Initiate, develop, implement, and revise patient care plans in collaboration with physicians and healthcare teams
  • Perform comprehensive patient assessments (OASIS), including medication reconciliation
  • Provide and coordinate high-quality clinical care according to the established plan of care and infection control standards
  • Supervise and guide Home Health Aides (HHAs) and Licensed Practical/Vocational Nurses (LPNs/LVNs), including conducting required supervisory visits
  • Deliver skilled nursing care to post-acute, assisted living, and nursing home patients in home settings
  • Accurately and promptly document patient visits, ensuring compliance with agency policies and payer requirements
  • Maintain timely documentation synchronization per organizational policy
  • Communicate effectively with patients, families, physicians, and care teams to ensure continuity of care

Qualifications (Required)

  • Active and valid Registered Nurse (RN) License – Massachusetts
  • Experience in home health or related clinical settings
  • Strong clinical assessment and documentation skills
  • Ability to comply with regulatory and infection control standards

  • Ability to work independently and autonomously
  • Fast learner with the ability to quickly onboard and adapt
  • Experience with post-acute care, assisted living, or nursing home patients
  • Strong time management and organizational skills

Additional Details

  • Mileage Reimbursement: Yes (between patient visits)
  • Travel Requirement: Local travel within assigned coverage area
  • Dress Code: Business casual (no jeans; no specific scrub color required)
