Job Title: RN - Home Health

Location: Salem, OR

Duration: 05 Months possible contract extensions

Work Schedule: Days 8:00 AM -4:30 PM

Weekly Hour Guarantee: 40 hours

 
On Call/Weekend Requirement: Yes

Coverage Areas: 60 Mile Radius from Salem

Will Milage be reimbursed between visits, from first patient to last patient (Y/N)? Y

What are the top responsibilities for this position? (Please be as detailed as possible):

• Perform Start of Care (SOC), Resumption of Care (ROC), recertification, and evaluation visits in patient homes
• Provide ongoing skilled nursing visits based on physician orders and individualized plans of care
• Conduct comprehensive patient assessments, including physical, psychosocial, and environmental factors
• Deliver skilled clinical care, including wound care, medication management, and patient education
• Accurately document all visits, assessments, and outcomes in the HCHB electronic medical record
• Monitor patient progress and communicate changes in condition to physicians and care teams
• Educate patients and caregivers on disease processes, medications, wound care, and safety strategies
• Coordinate care with interdisciplinary team members to support quality outcomes and prevent rehospitalization

Type of certification required: CPR , RN License and Drivers License
