Registered Nurse (RN) – Home Health

Weekly Hour Guarantee: 32 hours

Productivity Expectation: 30 points

Location: Uniontown, OH

Coverage Areas: Portage, Medina, and Summit County

Mileage Reimbursement: Yes (From first patient to last patient)

Shift Schedule: Mon–Fri 8:00am–4:30pm (Full Time)

Dress Code: Black, Grey, or Navy Blue Scrubs

Pay Rate: $45-$47/hr

About the Role

We are seeking an experienced Registered Nurse (RN) to provide high-quality home health care to patients throughout Portage, Medina, and Summit Counties. This role requires a clinician with strong autonomous judgment and a diverse clinical skill set. As a vital member of the field team, you will implement the nursing process to address the physical, safety, and emotional needs of homebound patients, ensuring compliance with all clinical standards and physician orders.

Key Responsibilities

• Clinical Excellence: Deliver skilled nursing interventions, including wound care, wound vac management, ostomy care, blood draws, PICC/IV care, and Foley care.
• Patient Assessment: Perform comprehensive assessments, including OASIS documentation, to develop and update individualized plans of care.
• EMR Proficiency: Maintain accurate and timely documentation of all patient interactions and clinical notes using Homecare Homebase (HCHB).
• Education: Provide critical teaching to patients and caregivers regarding medication management, disease processes, and specialized home care techniques.
• Care Coordination: Collaborate with a multidisciplinary team and physicians to report changes in patient status and ensure a holistic approach to care.
• Adaptability: Manage a 30-point weekly productivity expectation while remaining receptive to ongoing education and changes in the field schedule.

On-Call

• On-Call/Weekend Requirements: 1 weekday evening per week (4:30pm–8:00am) and 1 weekend per month (Friday 4:30pm–Monday 8:00am).

Required Qualifications

• Experience: Proven experience in Home Health nursing is required.
• Systems: Proficiency with OASIS and Homecare Homebase (HCHB) is mandatory.
• Certification: Current CPR certification required.
• Licensure: Active Registered Nurse (RN) license.
• Attributes: Personable, easy to adapt, and possessing strong clinical skills in wound and IV care.
