Description

American Traveler is seeking a Physical Therapist for a Home Health position in New Hampshire, requiring a minimum of 1 year of PT experience and a valid NH license.

Details

• Home health physical therapy setting with a caseload of 6 patient visits per day
• Day shift schedule, Monday–Friday, 8:00 AM–4:30 PM (5x8)
• Occasional weekend visits may be required as needed
• No call or float requirements
• Epic EMR is used for documentation
• Candidates located within 50 miles of the facility are accepted at a local rate

Requirements

• Minimum 1 year of physical therapy experience required; home health experience preferred but not required
• Valid NH state license required; candidates with a pending license will be considered if licensure can be obtained by start date
• BLS or HeartSaver CPR equivalent (AHA or Red Cross certifications only) required
• Manual therapy certification preferred
• Experience with orthopedic, neurological, and cardiopulmonary patient populations preferred
• Experience with home health documentation including OASIS and Plan of Care preferred
• License required if applicable

Additional Information

• Responsibilities include patient evaluation, treatment planning, gait training, therapeutic exercise, and patient/family education across a variety of orthopedic, neurological, and cardiopulmonary conditions
• 10-hour on-site orientation provided
• First-time travelers are welcome to apply
• Any color scrubs are acceptable
• Candidates who have previously worked at any facility associated with Solution Health are not eligible for this position, regardless of length of separation
• Two professional references required; may begin consideration with one reference on file and one pending
• All time-off requests must be disclosed upfront
• A facility-required drug screen must be completed within 14 days of the assignment start date
