Description

American Traveler is seeking a Home Health Physical Therapist with at least 1 year of experience to serve adult and geriatric patients across South Carolina.

Details

• Home Health setting covering Barnwell, Aiken, and Edgefield counties in South Carolina, with most patients located in Aiken or Barnwell
• Scheduling aims to group patients geographically to minimize travel, typically 20-30 miles per day with a maximum of about 70 miles
• Day shift schedule, 5x8 hours, 8:00 AM to 4:30 PM
• No weekend shifts required
• Average daily census of 175 with approximately 6 visits per day and full-time productivity around 25 units
• Floating within facility and scope of practice as needed
• Uses Home Care Home Base (HCHB) for documentation, transitioning to Epic around September 2026
• Mileage reimbursed at IRS standard rate, tracked through HCHB/Epic

Requirements

• Minimum 1 year of home health physical therapy experience required; will consider 6 months, though 2 years preferred
• BLS certification required
• South Carolina state license or compact PT license required; license required if applicable
• Experience treating joint replacements, COPD, CHF, and Type 2 diabetes preferred
• Experience with HCHB strongly preferred; those without HCHB experience must complete a paid online course prior to starting
• Comfortable working with adult to geriatric patient populations
• Strong skills in assessment, gait training, therapeutic exercise, and patient/family education

Additional Information

• Responsibilities include evaluation, treatment planning, and documentation for home health patients with orthopedic, cardiopulmonary, and neurologic conditions
• Any scrub color accepted
• Maximum 7 days of RTO per contract, must be finalized prior to start with no requests during orientation week
• Holiday coverage required based on how many major holidays fall within the assignment period
• First-time travelers accepted
• Local candidates accepted at the same rate
• Travel experience required
