Key Responsibilities

• Provide physical therapy services to home health patients in accordance with physician-approved care plans
• Complete and maintain OASIS assessments as required for home health documentation
• Chart and document patient progress accurately and in a timely manner
• Float to other locations as needed based on operational needs
• Collaborate with interdisciplinary team members to ensure continuity of patient care

Required Qualifications

• Active Physical Therapist license; Virginia state license required to be in hand at time of submittal or proof of compact license
• Home Health experience required
• OASIS experience required
• Specialty experience within the last year required
• SSN required at time of submittal
• One supervisory reference required at time of submittal
• Local candidates are welcome and accepted at the same rate

Preferred Experience / Skills

• Experience with EPIC or similar electronic health record systems
• Strong communication and interdisciplinary collaboration skills
• Ability to manage a flexible schedule, including floating assignments as needed
