Description

American Traveler is seeking a Physical Therapist for a Home Health assignment requiring 1+ year of recent experience and proficiency with HCHB and OASIS documentation.

Details

• Home health setting serving a geriatric patient population
• Caseload of approximately 5-7 patients per day
• Schedule is Monday through Friday, days, with optional weekends
• On-call required every 6th weekend
• Territory covers up to a 60-mile radius from the hub office
• EMR used is HCHB (Homecare Homebase)

Requirements

• Minimum 1 year of recent PT experience required
• BLS certification required
• OASIS experience required for completing Start of Care documentation
• Proficiency in HCHB required
• Active driver's license and personal transportation required
• License required if applicable

Additional Information

• Responsibilities include timely documentation and communication of patient care
• Traveler must be flexible with assigned territory
• Scrubs required, any color, no jeans permitted
• iPad provided; personal cell phone not required for work use
• Mileage reimbursed at the IRS standard rate
• Monday orientation with billable training modules completed during this time
• First-time travelers will not be considered
• Local candidates are not accepted
• Former permanent employees must be separated for at least 6 months and approved by HR before returning as a traveler
• Travel pairs accepted, but each traveler is assigned to their own territory and must have their own transportation
• Requested time off must be submitted at the time of application
