Assess patients’ nutritional status and identify appropriate nutrition diagnoses
Develop, implement, and evaluate individualized nutrition care plans
Provide nutrition education and counseling to patients, families, and staff
Interview patients regarding dietary habits, cultural preferences, and food needs
Review screening information and prioritize high-risk patients
Recommend therapeutic diets and address discrepancies in diet orders
Evaluate patient tolerance and response to therapeutic diets
Provide follow-up assessments and modify nutrition plans as needed
Document assessments, interventions, education, and care plans in the medical record
Coordinate with diet clerks to ensure accurate diets are served
Communicate with physicians regarding nutrition-related concerns or irregularities
Make nursing-unit rounds to identify patients at nutritional risk
Collaborate with physicians, nursing, pharmacy, social services, and other departments
Maintain patient data needed to recommend or modify therapeutic diets
Assist with monitoring clinical performance indicators and departmental quality initiatives
Apply age-specific growth and development principles when assessing patient care needs
Perform other duties as assigned
