Responsibilities
- Provide excellent patient care by coordinating the case management program from pre-admission through post-discharge, partnering with the patient, family, and the care team to optimize outcomes.
- Act as a liaison among the patient, family, nurse, physicians, and multidisciplinary team to ensure appropriate resources and timely progression through care.
- Serve as a consultant to the healthcare team on specific patient items, identifying barriers and recommending care approaches.
- Coordinate a continuum of care for a defined population, ensuring efficient pathways from admission through discharge and post-discharge planning.
- Document, report, and contribute to the ongoing development, revision, and reporting of the case management program.
Requirements
- Active registered nurse (RN) license as required for practice
- Experience in case management, care coordination, or discharge planning preferred
- Strong communication and collaboration skills with multidisciplinary teams
- Ability to work in an inpatient/acute care environment and manage multiple cases
- Knowledge of care transitions and discharge planning processes
