Responsibilities
- Provide inpatient case management nursing services, coordinating care across the multidisciplinary team
- Conduct comprehensive patient assessments and develop, implement, and monitor individualized care plans
- Facilitate efficient discharge planning and coordinate post-acute services, including referrals and necessary supports
- Communicate effectively with physicians, social workers, case managers, and other care team members to ensure seamless patient transitions
- Document patient progress, care plans, and discharge instructions in the medical record per policy and regulatory standards
Requirements
- Inpatient case management (IP CM) experience as a registered nurse
- Demonstrated ability in complex discharge planning and care coordination
- Ability to work in a hospital setting and collaborate with multidisciplinary teams
- Strong communication, organization, and problem-solving skills
- Willingness to participate in a travel assignment with the described schedule
