Responsibilities
- Provide excellent patient care by assisting in the collaboration, development, implementation, revision, and reporting of the case management program.
- Act as a liaison among the patient, family, nurse, physicians, multidisciplinary team, and patient’s healthcare benefactor to optimize outcomes.
- Serve as a consultant to the healthcare team on specific patient items within the case management scope.
- Coordinate a continuum of care for a defined population from pre-admission through post-discharge.
- Help ensure patients and families proceed efficiently through the course of hospitalization and transition planning.

Requirements
- Active Registered Nurse license (California) or ability to obtain per assignment requirements.
- Experience or exposure in case management, care coordination, discharge planning, or related area preferred.
- Strong communication, collaboration, and advocacy skills; ability to work effectively with a multidisciplinary team.
- Ability to operate in a hospital setting and navigate complex patient pathways within a travel assignment.
- Education: Graduation from an accredited nursing program (diploma, associate, or bachelor’s degree).
