Responsibilities
- Coordinate health plan care management for assigned members, including both inpatient and outpatient care planning
- Develop and implement comprehensive care plans; promote continuity of care and cost effectiveness
- Perform utilization review and care coordination; identify gaps and ensure appropriate service delivery
- Collaborate with multidisciplinary teams to integrate services and optimize outcomes
- Document care plans, progress notes, and outcomes in alignment with policies
- Communicate with members, families, and providers to support adherence and health outcomes
Requirements
- Active RN license in California
- Education: Graduate of an accredited nursing program
- Preferred: experience in case management or health plan care management
- Availability for 40 hours per week, day shift
- Ability to work in an inpatient/outpatient care management environment and participate in a travel assignment
